dc.description A variety of protective measures are being taken to protect vulnerable groups who are at high risk of severe illness from COVID-19 in all countries included in this review.  In general, people who are of an older age, people with chronic conditions if not well-managed or those who are in receipt of therapies that weaken the immune system, are considered to be vulnerable to more severe illness from COVID-19.  Belgium, Canada, Finland, Germany, Sweden, Switzerland, and Wales have a single grouping for vulnerable people. Australia, England, Northern Ireland, Norway and Scotland distinguish between those considered at ‘highest risk’ and those at ‘increased risk’. Denmark stratifies according to age groups and the US stratifies according to the certainty in the evidence underpinning each risk group.  Some countries have published additional risk factors that increase risk, although these do not always necessitate inclusion in the vulnerable group category. These risk factors include ethnicity, smoking, high BMI and pregnancy in conjunction with another condition.  Advice for vulnerable groups typically includes stay at home, practice hand hygiene, physical distancing and avoid contact with others. As some countries eased restrictions, the advice for people in the vulnerable groups changed to include the choice to go outside. For some countries this has included meeting people outside of their household, including physical contact with grandchildren.  According to the most recent update of the ECDC country level risk assessment (23 October) the overall epidemiological situation is assessed as stable for Finland and Norway with a moderate risk for vulnerable individuals. Although Norway has since been experiencing an increase in transmission and has responded with stricter protective measures for the general population and updated advice for high risk groups is expected. For the entire UK, Belgium, Denmark, Germany and Sweden, the overall epidemiological situation is assessed to be of serious concern; that is there is a very high risk for vulnerable individuals. Belgium continues to advise high risk groups to stay home as much as possible, England and Germany have reintroduced restrictions while Denmark, Scotland, Sweden and Wales have advised their high risk groups to follow general advice.  England, Northern Ireland and Wales have recently updated their highest risk groups to include adults with Down’s Syndrome and adult patients with kidney impairment (Stage 5 Chronic Kidney Disease). England has also added adults with problems with their spleen, for example splenectomy (a removed spleen).en_US
