dc.description | Welcome to the Operational Plan for Community Healthcare Dublin North City & County. Community Healthcare is the term used to describe the range of health and social care services provided by the HSE outside of the acute hospital system. Community Healthcare Organisation Dublin North City and County (CHO DNCC) is one of nine CHO’s across the country and is responsible for providing care services to a population of 621,2161 in Dublin North City and County. A key priority for 2017 is to progress the further development of the structures and processes intended to ensure that the CHO area achieves high quality integrated services as close to home as possible for the people of Dublin North City & County. We have established clear pathways on integration across community, hospital and residential services to ensure clients receive quality care in the most appropriate setting with straightforward access. This will be a cornerstone in our progression of services for 2017. | en |
