| dc.description | Sian Abhaile was targeted at older people who, in spite of their wish to remain living in their own homes, might be at risk of having to move into residential care in order to have their long-term care needs met. Sixty-two (62) older people received Sian Abhaile services during the first 18 months of the project. Of these, the average age of participants was over 82 years, women outnumber men (40 women, 22 men) and service recipients' needs ranged from moderate to high physical dependency with a significant minority with dementia related or other mental health problems. 37 older people were facilitated to leave hospital with Sian Abhaile support services; 25 clients were referred from the community and these included 12 people who were referred by the community nurses, 6 by the Psychiatry of Old Age service, 4 by the day hospital in St.Vincent's University Hospital, 2 by the hospital Out-Patients Department and 1 by the community social worker for older people. | en |
