dc.description	Primary care is the first of the six frameworks for change set out in the National Health Strategy, which seeks to rebalance the emphasis from secondary care to primary care. Between 90% to 95% of the health and personal social service needs of our population can be delivered in a primary care setting. These services provide first level contact that is fully accessible by self-referral and have a strong emphasis on working with communities and individuals to improve their health and social well-being.	en
