dc.descriptionThe purpose of this guide is to provide a practical support to assist healthcare teams working in older person services to continue to develop and sustain person-centred practice using Practice Development (PD) processes. The guide has six sections, beginning with an introduction to the strategic context and an overview of the Older Person Service National Practice Development Programme 2007-2009. Section two explores the concept of personcentred care and PD as a model of delivery. Section three discusses facilitation skills which are integral to promoting change and enhancing practice through PD. In section four, information is provided on how to collect evidence from practice using evidence based PD tools and processes. In section five, the process is continued with an overview of action planning and evaluation phases. These are reliant on utilising and developing facilitation skills and PD methodologies.
