Community Healthcare West
This operational plan outlines our priorities and goals within Primary Care, Health and Wellbeing, Older Persons’ Services,
Disability Services, and Mental Health services which will be delivered using an integrated approach to provide a
sustainable, high quality, safe and effective health and social care system that meets the needs of our population. Our
Operational Plan sets out the type and volume of healthcare services to be provided in the Community during 2021. It also
includes reform actions that will be undertaken during the year to continue to improve and expand upon service delivery in
the spirit of the Sláintecare Implementation Plan to deliver person centred care within our allocated funding.
The National Service Plan for 2021 reflects a very significant investment increase for health services. This unprecedented
national investment has created a real opportunity for us to improve the experience, nature and variety of services and
care options that we provide to the people we serve. We will continue to implement significant service reforms that have
been planned for many years. Notably there will be a demonstrable shift in the provision of services from hospital to
community settings, there will be a greater emphasis placed on prevention, supporting people with chronic conditions,
access improvement and service delivery close to or in the home i.e. Home First.

In line with the Future Health and Sláintecare Reports, the ultimate aim of the health service is to meet the vast majority of the population’s health and social care needs in the least complex setting, with residential and hospital-based care being reserved for only those individuals requiring specialised and emergency care. These models of delivery are deemed more convenient for service users, supports people to self-manage and live more independently, offers better value for money and facilitates greater service integration and proactive delivery of care.

Community Healthcare West
This 2019 Annual Report is an acknowledgement and
celebration of the work achieved in that year. Each year
our Organisation produces an Operational Plan which we
strive to deliver amidst the many service challenges we
face. This Report supports our obligation to be accountable
to our many stakeholders as we implement
recommendations and goals laid out in the Sláintecare
Implementation Report.
With a budget in excess of €500m, the Senior Management
Team of 2019 oversaw the delivery of a complex and
extensive range of services as outlined by our Performance
Indictors (Appendix 1). Our Organisation also ensured that
our intention to reform services and service delivery was
supported via an application to the Sláintecare Integration
Fund, with an allocation of €2million for nine pilot projects.
Significant progress regarding the assurance of Quality and
Safety of our services was achieved. This was done by
improvements to incident reporting, mapping of Infection
Prevention and Control data, oversight and governance.
The Community Healthcare West Healthy Ireland Plan
(2018-2022) continued to support interagency
collaboration for health promotion and improvement.

The Mental Health Services has implemented an action plan in response to an external review of Roscommon Mental Health Services. A recommendation in the plan included developing a nursing document to focus on the vision, values, principles of nursing and caring. This was the start of our journey to our vision document.
