This report presents the findings of the investigation by the Health Information and Quality Authority (the Authority or HIQA) into the governance and assurance arrangements that the Health Service Executive (HSE) has in place to ensure the safety, quality and standard of services provided to patients in the Midland Regional Hospital, Portlaoise (Portlaoise Hospital), Co Laois. On 30 January 2014, the RTÉ Investigations Unit broadcast a Prime Time programme about the tragic deaths of newborn babies in Portlaoise Hospital and the subsequent management of patients and their families by the hospital and the HSE. Following the broadcast, the then Minister of Health asked the Chief Medical Officer of the Department of Health to conduct a preliminary assessment of perinatal deaths and related matters from 2006 up to that point in 2014 in the maternity services at Portlaoise Hospital.
