Description:
Chronic Obstructive Pulmonary Disease (COPD) is responsible for approximately 210 admissions per year to SVUH and over 3000 bed days. Nationally, 40% of patients admitted with an Acute Exacerbation of COPD (AECOPD) are readmitted within 90 days of discharge. Internationally, a number of interventions delivered  prior to discharge which have been shown to improve COPD care, have been grouped into a Discharge Bundle of Care and have been implemented in a number of hospitals. This audit evaluates to what extent these interventions are carried out in SVUH
