Heart failure is considered as an age related problem in most countries, increasing in prevalence and incidence, with significant mortality. Recent models of end-of-life care have been mainly developed for cancer patients. Discussing end-of-life issues with heart failure patients is challenging. This review is focused on advance care planning discussion with heart failure patients. Advance care planning can be helpful in a situation where patients are no longer able to make decision or communicate their wishes.
