Description:	The Housing Preference and Assessment Survey is presented here. It was designed to help service users to express their housing need, housing supports, preferred housing and preferred supports. It can be used as both a survey and individual assessment questionnaire. Mental health services can change or adapt it to suit their own particular situations.
